Case Management & Discharge Planning

At Newberry Hospital, we believe that exceptional healthcare involves more than just meeting physical needs. Exceptional care also extends to the service you receive once you leave our hospital.

The members of the Social Services Department who handle case management and discharge planning are an important part of your healthcare team. We work with patients and family members to provide emotional support during time at the hospital. We also share information concerning community programs that provide special and/or ongoing support for the patient and family. Often this support extends beyond the walls of Newberry Hospital and involves other helpful community resources. 

Discharge Planning

Going home from the hospital after an illness can be stressful for patients and family members. Our goal is to make this transition as smooth as possible. 

Before the physician discharges a patient from the hospital, a multi-disciplinary medical team meets to develop an appropriate discharge plan. This plan takes into account the patient’s medical and social needs. 

The team includes a nurse, a social worker, and a representative from Dietary Services, Respiratory, Behavioral Health, a UR nurse, Physical Therapy, and Pharmacy as required.
