Registration Process Guide

Below is a guide to help ease and explain the process of registration at NCMH. If you are interested in a list of insurance companies accepted by NCMH, click here.

INPATIENT REGISTRATION
Please check in at the Inpatient Admissions desk, which is located off the front lobby. You will be directed to a registration booth, where a representative will collect information, such as your name, address, and insurance/payment information. You will be asked to sign various admissions forms.
Please bring the following:
 All active insurance cards
Picture ID
Living Will/Healthcare Power of Attorney,etc., if you have one
Please leave your valuables at home for safekeeping.
OUTPATIENT SERVICES
For Lab, X-Ray/Diagnostic Imaging, Stress Test, you should check in at the Lab/X-Ray Registration desk.
For MRI, you should check in at the MRI Department in Atrium Professional Center located in front of the hospital.
For Surgery, you should check in at the Fulmer Surgery Center desk located off the left side of the front lobby.
After you have checked in, you will be directed to a registration booth, where a representative will collect information, such as your name, address and insurance/payment information. You will be asked to sign registration forms.
Please bring the following:
All active insurance cards
Picture ID
Physician's Orders
Please arrive for services at least 15 minutes prior to your appointment time. To reduce your wait time on the day of your appointment, a registration representative may call you before the day of your test to ask you questions necessary to begin the registration process or you may pre-register online by clicking here.
Payment for services is expected at the time of service. Please call for more information.
